Project Overview
Between May and October 2025, Remian Diagnostics Limited successfully implemented a community-driven, digitally enabled adolescent sexual and reproductive health (ASRH) program in Trans Nzoia County.
The project addressed critical gaps in youth-friendly SRH access, confidentiality, continuity of care, and data-driven decision-making by combining offline-capable digital tools, trained community health systems, and remote clinical support.
The intervention significantly improved adolescents’ access to accurate information, early risk identification, counseling, referral services, and follow-up care—while reducing stigma and barriers to care-seeking.
Problem Addressed
Adolescents in Trans Nzoia County face persistent sexual and reproductive health challenges, including:
High rates of teenage pregnancy
Sexually transmitted infections (STIs)
Limited access to confidential youth-friendly services
Stigma, fear, and cultural barriers to care-seeking
Inadequate follow-up and referral systems
Shortage of adolescent-focused healthcare providers
Weak SRH data systems for programming and accountability
Prior to this project, many adolescents relied on unsafe information sources or delayed care, leading to preventable health and social consequences.

To improve access, confidentiality, and quality of adolescent sexual and reproductive health services in Trans Nzoia County through digital health innovation and strengthened community-based care systems.

1. Offline Adolescent SRH Digital Health Platform
A secure offline-first mobile and desktop health application deployed to community health volunteers, nurses, and peer educators to:
Conduct confidential adolescent SRH assessments
Capture menstrual health data, STI symptoms, pregnancy risk, and psychosocial indicators
Use color-coded clinical alerts (Red / Yellow / Green)
Store adolescent health histories securely
Sync data to a clinician dashboard when the internet is available
Enable remote clinician review and guidance
Deliver confidential SMS reminders for clinic visits, medication, and counseling follow-ups
2. Youth Sexual and Reproductive Health Hotline
A confidential SRH hotline provided:
One-on-one adolescent counseling
Support on contraception, STI symptoms, menstrual health, and GBV
Direct linkage between adolescents, CHVs, nurses, and clinicians
Emergency escalation for sexual violence and high-risk pregnancies
Tracked referrals and follow-ups
